This weapon is located on top of a tower close to the beginning of Castle Ensis. You can reach the castle if you follow the road north through the Gravesite Plain and cross the Ensis Greatbridge. But be careful! The Ballista is located at the back of the bridge.

Once you pass that point, you'll soon see the Castle Front Site of Grace. Ascend the stairs, defeat the Troll Knight, and go up the next set of stairs to the left. Keep going left until you reach a couple of guards and a dog. After you kill them, look to the western corner, and you'll see a ladder that leads up to the top of a tower.
