In Episode 5 of The Boys Season 4, we saw the actual effects of the Supe-Killing virus we've been hearing about. After Victoria Neuman's scientists injected different animals with Compound V, some of them became completely uncontrollable and went on a killing spree.
While trying to escape from the V'd-up sheep, Butcher found one of the test rabbits in a bush. The rabbit, however, suffered consequences from Compound V that we've never seen before. A tentacle creature tore out of its body, trying to attack Butcher. If you are as shocked as we were, keep on reading!

Episode 5 gave us a lot of important details that will be significant for the plot from now on. The laboratory located on Stan Edgar's farm showed us exactly what happens to organisms after they have been injected with Compound V. You already know where we are going with this.
So far this season, we have been seeing that something like a worm is clearly living under Butcher's skin. After seeing that rabbit, we already know what it is. When Butcher killed Ezekiel, it was clear something was seriously wrong with him. He was completely unconscious the whole time, and there was no way he managed to kill him with superpowers like that.
It is safe to guess that Butcher has a tentacle monster, just like the rabbit's, living inside him. It is still weird to imagine how it managed to come out of him and return without leaving any trace, but we will learn more about it later for sure.

The main reason why we are so sure that Butcher has a monster living inside him and is caused by Compound V is because of the animated show called Diabolical (2022). Although the show is not considered canon to The Boys, there are some episodes that have been confirmed to be accurate for the main series as well.
In Episode 7 of the animated show, which is among those considered canon, a husband injects his dying wife with Compound V to save her. However, unlike what we have seen in The Boys so far, the wife doesn't get superpowers; instead, the compound turns into a tentacle monster that can leave her body. Sounds familiar?

It seems that HBO decided to go the same way as Diabolical but gave the destiny of the old couple to different characters in The Boys. We saw Hughie in a similar situation. His father got injected with Compound V on his deathbed, and while he did not get a monster, he went totally berserk. On the other hand, we saw the V'd-up rabbit that resembled what Butcher's been experiencing so far. At this point, we are convinced that by the end of Season 4, we will see Butcher with the same tentacle monster.
